LOL. Quick fix.
It does not appear that your flash hider is indexed correctly. There is a "T" for top on 1 of the flats. Indexing is important for control of muzzle flip. If it does not index to top easily flip the crush washer over, sometimes that helps to get the indexing with minimal torque to the flash hider.
