You are required to submit the invoice that details the model number you purchased. We mail two physical copies of the purchase invoice to our customers.

Yes cut the UPC and model number out of the packaging and submit it with your rebate form, along with a copy of your purchase invoice. Be sure to follow all of the instructions and requests on the form before submitting it.
