Bad ammo..

Or jammed or damaged firing pin, or excessive head space, or failed bolt lugs. I have several AKs here at the range that commonly pierce primers. Eventually the pieces of primers accumulate in the firing pin hole and jam the pin forward. The gun can then fire out of battery. I also have a gun with a cracked bolt lug and trunnion. So continued use could result is a breech explosion.

Funny thing is they were posted at the SAME INSTANT!!

I am locking this one because the GD thread has more exposure and everyone should wear eye protection regardless of what gun it is.
