I changed up my gun a bit and added a new charging handle and speed bar both by Dave's Metal Works. I got an oversized button safety but haven't had a chance to install it yet. Here's what it looks like:

 
I have a Mesa tactical 8 shot side saddle on my M4 for about 5000 + rounds with no issues. I prefer it over the Velcro versions personally, because the first gen of the Velcro seemed to be full of issues.

The new ones seem Ok, but I still love my Mesa tactical.

As an aside, what kind of issues result of the bolt on kind?

People over tighten the the bolt-on sidesaddles, Benelli receivers are made from aluminum so it will pinch the receiver, obviously more friction.
velcro puts zero side pressure on the gun so it makes it the most reliable. ideally a bolt-on side saddle the screws should be sleeved so it cannot put a side load on the receiver from over tightening. i am not sure if anyone does this however if a bolt-on side saddle is installed correctly it shouldn't be any problems.

The Mesa Tactical side saddles for the M4 all have an integrated rail, so they don't bolt through the receiver horizontally, there's no pinching/squeezing like on one that's mounted through the receiver crossways, such as the 870 ones.

The issue is that the the additional weight of a loaded side saddle and any heavy bumping/jarring can put stress on the screws.
I've never had an issue, but I also used loctite on the screws and under the rail, without over tightening and possibly stripping the screws. Another option for added strength is to put double sided tape (like 3M automotive) sandwiched between the shell holder and receiver.
 
Pick up a B & T front hand guard from Wolverines. It's just, well, beautiful to shoot. Far better than the stock plastic, though a little heavier, plus you can mount plenty of toys. Expensive but more than worth it in the end.
 
don't go with a solid metal or plastic one, benelli,s are famous for having issues with them
use a Velcro based side saddle, our 7 shot is a big seller for M2 and m4 owners

Just wondering if anyone has or has seen an Benelli M4 Extension Tube in stock anywhere here with our sponsors? Everyone seems to be out :( from what my searching has turned up.
