benelli supernova failure!

Today while out with some friends shooting some clays at the farm, one guy had a major failure from a benelli super nova. The gun is almost brand new. It has only be out 2 or 3 times...

What happened as far as we can tell is that a shell fired as it was being cycled into the chamber. As soon as it fired he dropped to the ground and so did the gun. I rushed over to make sure he was ok and then make his firearm safe. He was kind of disorientated and said somthing hit his chest and face. When I was making the gun safe I found that it was jammed 3/4 closed, and burn marks on the bolt and the ejection port going backwards down the gun. When I pulled harder an empty case popped out. Upon further inspection there was another case in the chamber with no brass on it, lining the chamber. Also we found a wad halfway down the barrel...

Any ideas?

As I previously said the gun is almost brand new..

Ammo was challenger first clas target loads, #8, 2-3 /4"

I was using the same ammo all day in my rem 11-87 autoloader, win 1200 pump, and my SxS break barrel. Also a third shooter was using it in a rem 870 with no issues...

Any ideas would be appreciated. Thanks in advance.

Wow that sounds like a terrible experience. I can't explain why that happened but I can tell you that I recently stopped counting and I have over 1800 rounds through my supernova with no issues. It has never jammed or failed to fire. Mainly federal target load but numerous other brands when hunting waterfowl.

You mean the plastic shell of a previous round was still in the chamber (like he extracted only the brass part from the previous shot) and another round was chambered and shot or did the shot went by itself while he was closing the action ?

If it's scenario 2, what does the inside of the action looks like ?

Never had any issue with mine neither BTW
 
Yes the plastic from a previous shell was still in the chamber... we never found the brass portion. As far as he says he never pulled the trigger on the second round, it was 3/4 into the chamber and went BOOM!

The inside of the action had a yellow and grey residue all over it. The gun is being disassemble tonight... so a further inspection will be done...

If the chamber was lined by another hull, it still does not explain why it fired. The shell should not of chambered and the gun should not have went off... in my mind anyways.

Its the strangest thing I have ever seen in a while!
 
Sounds like a manufacturing failure of some sort in the shell, i have heard they are pretty cheaply made, and will the new shell somehow in the chamber 3/4 or so it can still be fired if the trigger is pulled and enough that the hammer hits the firing pin. Im just surprised he didnt notice the action not move all the way in smoothly or tryed to force it and fired it anyways thinking it was all the way in. Id be taking pictures and contacting challenger with the lot number if there is one on that box.
 
I would not take the gun apart! Talk to the dealer were he bought it first and show them what happened! Just to cover your butt!!
