It's posted in our team forum so unless you are a team member you can't get in.I just got off the phone with them and was told the all the engineers, gunsmiths and senior gunsmiths wanted to take a look at, since it was such an odd occurrence. They measured everything and it all checked out - internal specs, firing pin protrusion. They were all stumped as to how it fired out of battery. The guy I spoke to said since they didn't know what caused it, they're going to replace the entire upper at no cost. There's about a 2-week wait on barrels right now, so I'll be running the class with a friend's A3 post-ban style 16" bbl. The same rifle I have, except there's no flash suppressor.

I was shooting UMC 45gr. JHP. I was only loading 20 to a mag since they are packed in 20 rd. trays. I had shot off 2 mags with no problem. The third mag had several failures to feed/fire. I wasn't sure if it was the mag or not since a simple "tap, rack, decide" cleared the problem each time. It happened about 4 times in that mag. My friend standing off to the side said it looked like the bolt wasn't seating fully in battery. After finishing that mag, I inserted the fourth one, firing a couple shots slowly. No failures, so I figured it was good to go. This was going to be the last mag of the day, so I started popping off rapid-fire shots. Bang-bang-banging right along, then BOOM! I feel a lot of gasses blow back on my face and hear the sproing of a mag spring sliding out.
I roll the rifle over and see the bolt stuck half way back and pieces of casing inside the chamber. The bottom of the mag is mushroomed out and the follower, floorplate, and ranger pull are stuck down in the dirt. There was no pulling the charging handle back to reseat the bolt. It was stuck good.
