Doesn't a 3 shot extension typically sit proud of the 18.5" barrel, is regular capacity with this mag tube 3 rounds?

The MCS is built to have it fit flush to the shortest 10 inch barrel / shorter mag tube.

The barrel lug would need to be moved on a standard 870 barrel to fit on the MCS. Can be done, from what I know a gunsmith shop in lower mainland bc has done it before for clients.
