Hey everyone, I have the CFSC forms and I filled out mine fine but filled my little brothers ID section with his health card number which I was told is not valid for use as his ID. The instructor said to use our mothers drivers licence number as he is under 18 so she did that but, we are unsure of who should sign the blank that says, "I hereby certify that the information in part A is true and accurate". Should my brother as he is the student or my mother because she provided the ID? I cannot call and ask as it is late and the course starts back up at 9 in the morning and if mom does have to sign it, it has to be before me and my brothers leave.
