"...ball park price on having one made?..." Machining runs over $100 per hour, plus the cost of heat treating. Any machine shop can do it, but you'd have to provide the drawing. Said drawing isn't just a picture. Requires dimensions and tolerances.
As mentioned Pacific Tool and Guage is the place to go. Go here and look for whatever reamer you're looking for. It'll be less expensive to buy or rent than have one made.

doesn't matter if the design is your own or one of their 8000 calibers the price is $139. pay the extra $15 for TiN coating. 8 weeks to get my last reamers. awesome customer service even if you have questions later. make sure to state pilot size though.
 
You can make your own, half-shell reamer for a one-off. I haven't done-it in a while but will need-it for .243 and .25 Hornet chamberings.

im no machinist. not by a long shot, plus the lack of tooling hurts my chances :D. ill have to buy.

i was playing with the idea of necking a 7.62x39 case and necking it down to .5.56mm to use common .224 bullets.
 
Sounds like you want a 220 Russian reamer. I had one made 10 years ago for Benchrest shooting . They are very accurate. Mine has a tight neck and was a bit of a PITA because of the angles (they are pronounced) so you get a little more bolt thrust.
I was using Lapua brass as well. You will find getting Dies and such will be a chore.
 
I do my own work if you search my posts you will see some of my work under extreme gunsmithing. I wouldn't suggest a 220 Russian , as I stated bit of a bear to work with unless everything is perfect.
