This failure mode is new to me.
I have always hand-tightened my decapper pins. Here's what happened - notice that the pin was wedged into the flashhole by a small piece of tumbling media.
The pin pulled out, and I didn't notice for a bit (was doing a size/decap run before case trimming / pocket uniforming). Nothing like filtering through a pile of brass to find the ones with the primers still in.
So now I tighten the decapper with a pair of padded pliers.
