Spring and follower and check the shell latches, I always upgrade with S&J’s delrin follower and their mag spring. You’ll have to trim the spring to size as it comes long, one spring will do a factory reg length mag spring and an longer spring for the +2 factory extension.
 
Look at the shell latches on the inside of the receiver. They're staked in place one on the left and the other on the right. These latches control the release and retention of the shells from the tube magazine as the bolt travels back. If there is a problem with either one of these latches, they can cause your gun to not release shells or release more than one shell.
 
Lazy stroking?
Any dimples in the mag tube?

I didn't think I was going easy on it, but I will try it again and pay closer attention to it. It's not here with me so I'm unsure if it has the crimp dimples.

It's never double released, and only fails once in awhile but it's super frustrating when it happens while bird hunting. I will be sure to check them out.
 
A issue with shell latches can cause the problem to be intermittent since even if the pop out of their staked points, the triggers pins will prevent them from completely falling out. I was trying to say that it can cause it to feed more than one round and not that it will. It depends on which shell latch has the problem and where it has the problem.
 
Is there spring tension on the shells in the tub...i have seen the spring get bound up around the plug when it is put in backwards
