Most factory brass has inconsistent neck thickness as formed right from the factory. Using a tubing mike to measure thickness at various points around a case neck, thickness can vary by 3-5 thou. Thus a bullet can start off down the bore, off-centre by that amount. Outside neck turning on a tight mandrel can true this up. You usually specify a tighter neck chamber, because the neck turning necessarily reduces the neck diametre, below SAAMI Neck dia. specs.

Uneven brass flow can be caused by a bad chamber, but in most instances,
it is brass that is not uniform in thickness.
Outside neck turning can help this to a certain extent.

In some cases, you have to outside turn to get the brass down to the correct thickness when necking down from a larger caliber , to match the chamber.
