GSG 1911-22 Weird Ejection Issue

Bought what appears to be a low mileage GSG for me wife and we are having an issue with it ejecting a live round at the same time as it ejects the spent case. I've ruled out being mag specific or her limp wristing it or anything she is doing as it has happened on multiple mage for me as well. Any thoughts or ideas on where to start?
 
I'd be interested to hear how you "ruled out" the issue being mag-specific unless it occurs with all mags that is (how many mags do you have?).

I ordered a second mag when I bought a GSG 1911 .22, and labelled them 1 and 2. Mag 1 worked flawless while mag 2 had issues. The only two things I'd suggest are: 1. check/reverse the orientation of the spring and 2. check/adjust how many spring loops are above the nanny rivet. I improved mag 2 performance somewhat by doing these steps. Good luck.

there shouldnt be a "nanny rivet" ... its the .22 1911 ... it only has 10 round mags ...unless im wrong ... been known to happen , i have 5 gsg mags and they all work fine , have you cleaned up the mags or modded them in any way ?
 
I ruled out the mag by the fact it has happened with 3 different mags over multiple shooters. I haven't touched anything at this point as we bought it used and it appeared to have a low round count when I pulled it apart, as far as mods the mags appear untouched.

OOPs on the nanny rivet comment, confused one other mag issue with the GSG, but the spring orientation comment does apply. You can see in the slot down the side of the mag if the spring looks similarly oriented on all mags. I'm not saying it will completely resolve the issue, but it seemed to lessen the issue when I did it.
