it states:
Have you been certified by the Chief Firearms Officer as meeting the safety training requirements OR have you successfully completed a course
approved by the Attorney General of Manitoba or Quebec prior to 1995?

Does that mean completing and passing the restricted and non restricted courses or something else? I ask because the two questions before it ask if I have "passed the Canadian Firearms Safety Course test?" and Restricted.

Are you sure this isn't a question for 'office use only'

It sounds like something your provincial CFO signs off on before it goes to the CFC.

it asks if you have done any of those tests that were done in before
if you just took your PAL or RPAL couse/exam the answer is no

I just filled that in a few weeks ago: Answer is "no" if you just took the course.

From what I read somewhere, if you had your course prior to 1995, your course was good enough for both restricted and unrestricted.

Nowaday, you need 2 courses, more money for the government ;P

I think its the CFSC 1995 to 1998 (December) is considered equal to both the CFSC and CRFSC. As the CRFSC did not exist till after December 1998. Or at least that is my understanding.

This is correct, I have an old CRFSC manual, it does covers both restricted and non-restricted.
