Another gun that has passed the NATO over the beach test is the SIG 516.
The reason it has done so, is because they made three modifications to the design, which they carried over to their M400.
1) They screwed a small nub into the inside of the barrel extension right where the extractor site when the bolt is locked into battery. This stops the extractor from blowing out, which is the weakest part of the breach end of the barrel.
2) They machined a slight bevel into the inside of the upper receiver where the cam pin sits. This allows the cam pin to begin rotating with slightly less force.
3) They drilled a few more holes into the indents in the underside of the lower receiver extension. This allows water to exit the extension faster and reduces the hydraulic effect of a water filled tube.

There is no NATO OTB test. It's a made up special requirement and was not promulgated by NATO.
The 416 has to be ordered modified for OTB, and it is modded much the way you describe for Sig. The key difference is that they all eliminate the chamber chamfer that is the last point of support so the case does not blow out. Unfortunately that last point of support was specifically designed by stoner to be the safety valve to protect the soldier in case of a breach obstruction such as a squib, cleaning rod, sand, etc.
Any rifle can be modified to pass this test, there is some debate as to whether they should or not.
The last time the US military chased it's tail looking for a new rifle nearly all the competitors submitted rifles that passed the OTB including Colt. The only thing that drained out of those holes was wasted money.
