I've never really needed to access it; if suppressors ever became legal, I'd have an excuse. Concerning the break, there doesn't appear to be any significant build up or wear on the end of the guard, so the gas is escaping as it should be; I suppose this would be a bigger issue on non-perforated handguards, especially carbon fiber ones. I would think putting your hand at the end of your guard could spell a blistered hand, though.
I do see how it could go terribly wrong, if there's not enough space (and if the brake is aggressive enough):
