I have dealt (purchased) custom holsters from the following 2 manufacturers in the U.S.

Lead time for a custom holster is VERY LONG, up to 6 months, but it is well worth the wait. The quality is light years ahead of the mass produced holsters.

Haugen is a two man operation, I had my M&P holster lined with Kangaroo leather and it is very gentle on the guns finish. I highly recommend getting holster leather lined to protect finish on gun. Cost $15-$25 dollars more and you won't have to re-blue your gun in six months!
