I came across this useful tip on the 1911 Forum after discovering that my Lee ACB was throwing inaccurate charges while loading .38 Spec. Apparently this is not unusual when throwing very small charges of pistol powders (<4 grains.)
Here's another article showing the same mod. This guy used epoxy for a more permanent/durable modification.

I reduced the opening about the same as in the bottom pic and have found it throws 3.8gr. of 231 very consistently now.
This is a reproduction of a fix for the problem that occurs with the Lee Adjustable Charge Bar because it does not properly align with the Powder Hopper. By placing card stock (I used index card pieces) as a buffer or shim for the far end of the hole you get a much more efficiently accurate charge.
After 15-20 charges, I have not had any deviate more than 1/10th of a grain. Usually I didn't even have that happen.
