I'm new to lever guns and have recently bought one. I have some unfounded concerns about the method of loading these rifles (eg, pushing the rounds through the loading gate and into the tube with the bullet end of one cartridge against the rear (primer) end of the cartridge ahead of it.

Has there been any known instances of accidental discharge due to loading in this manner?

That's why ammo intended for lever rifles has the 'flat point' profile on the bullets. So the ammo in the mag tube doesn't set off the cartridge ahead of it under recoil.

You aren't supposed to load 'spitzer' [i.e. pointed] bullets into cases like the .30-30 [if you reload] if you are going to use the tube magazine. Single shot only! ;)
 
I am unsure about accidental discharge, but remember not to use pointed bullets in a tube fed gun, they should be flat points.
 
Hornady red tips

There is however some new bullets being made by Hornady with their trademark red plastic tip which are pointed and intended for use in lever guns
 
I've never heard of it myself. I'm sure it has happened at some point and time. If it does happen it must be something that happens every two blue moons or so. Considering the number of lever guns being used designed to be loaded in this manor. I wouldn't worry about it much! If you are then go to a BLR.

Blunt roundnose or flat points work ok, but as been said by others, stay away from pointy boolits. Primers that project out rather than being slightly recessed or flush are another issue (good idea to check any ammo you use to make sure this is NOT an issue).

As for magazine kabooms, haven't witnessed any, but heard those that have happened mostly involved pointy boolits.

Ditto in what was said above regarding spitzer(pointed) bullets. They are not to be used in a tube magazine. The only spitzer that can be used in a tube magazine is the new Hornady Leverevolution bullets that have a soft, deformable flex tip on the tip of the bullet. The flex tip will deform when pressed against another cartridge, thus forming a shock absorbing barrier against accidental detonation in the magazine.
