Looking For Help

I have a close friend who has a shot gun given to him. It is sentimental for him. He has asked me to help find a couple of parts for him.
It is a
Stevens model 58D-12ga
Savage arms
West Field Mass U.S.A.
Proof Tested 12 Gauge
2 3/4 inch chamber.
Also has a
This is what was on the barrel. Does anybody know around what year it maybe, and if it was a half decent shootgun, not that he cares, as it has memories for him.
He is looking for a 3 shoot clip for it, and holder, and also a complete bolt as it is a little sloppy. It will shoot, but is only a single shot as no clip. Anybody that can help would be much apperciated.
