Lost paperwork, help!!

Hey guys,
So I passed my restricted and non restricted firearms tests. Got my test results in the mail, FINALLY!
Now I got the application for the two licenses from my instructor when I passed. Here's my problem, my mother decided to try and be nice and clean my room for me. she accidentally recycled them...

Can I get another one? Can you print them off online? Where can I get one?!

Dude, you just saved my ass! Thanks so much!! I'm filling it out now so I can mail it in tomorrow.

But I have another question! I'm confused by one part of the application. I did my test for my Restricted and Non-Restricted firearms licenses and passed. On the application it asks

"Have you completed a canadian firearms safety course?" I haven't done the course, but I passed the test, do I check yes or no?

"have you completed a canadian restricted safety course?" Same deal, I haven't done the course, but I challenged it and passed the test!

"Have you been certified by the Chief Firearms Officer as meeting the safety training requirements OR have you successfully completed a course
approved by the Attorney General of Manitoba or Quebec prior to 1995?" Ugggghhh.....what!? Does this mean I challenged the course and did the test and a chief firearms officer has certified me as passing? (and I mail in a photo copy of my test results?)

No, not inless you had a FAC pior to 1995. Their is probly more to it then that as well, perhaps an older gentlemen would elaborate more on this?
