Need a Polish translater / collector

Just aquired this interesting certificate. I'm not able to read the script, but Polish comes to mind right away.

The paper has been profesionally framed at some point to protect it. Looks pretty fragile. Crease marks in it so was probably carried for years in a wallet. Unit photo added at the bottom, maybe covering something else? The back has been profesionally papered and sealed so I can't take it out to examine any further.

Anyone seen one of these or can translate?
