Not even on paper at 25 yards

Sounds silly, but make sure the rings both have the clamp screw head on the same side. My BIL did not think it made a difference, and could not hit a target at 10 yards.

Everything "looks" to be correct. Rings etc seem to be installed correctly (symmetrical ) I'll take everything off and inspect when I get home from work.

I would install another scope (if you have one) on this 22 to figure out what is going on. If you don't have another scope, take this original scope and install it on another 22 (if you have one) and see what you get.

So. I managed to get the scope and rings off.

I couldn't get the two small rail pieces off though. We'll, I'm sure I could of, but the rails are held on my two screws each and we're very tight. I was afraid to apply too much pressure. I did give it a good shot though.

Are the two rail pieces installed from the factory?
