I've had it hapen twice in the same day in my 44 mag revolver. I don't tumble my cases, so it wasn't moisture or media stuck in the case. The primer went off with a fair bit of force because it jammed the cylinder into the frame via the primer comming out of the case a bit, and then not having the back blast to driver the case backward to reseat the primer. My powder was brand new off the shelf. While beating the bullets back out of the barrel, the unburnt (but very discoloured) powder was going everywhere, including into the trigger works, and jammed the cylinder rotator up so it wouldn't come out and turn the cylinder. I took the gun apart, and let me tell ya. It's a bit of a pain getting all of the grains out of everything. Powder was WIN 231 and I was using large magnum pistol primers made by Winchester. I know the flash hole was clear because I use a progressive press. The the step right before the primer goes in is running the decapping pin though it to get rid of the old primer. Perhaps some junk that was left in the primer pocket got into the flash hole and created enough of a restriction that it didn't flame into the powder enought to properly ignite it??? Who knows.
