the issue is defiantly with the Dillon die specifically the primer punch its too fat and will allow the old primer to stick to it and draw it back into the old brass on the up stroke, easy fix is to grind the pin down to make it smaller around and that will fix 99% of he issue...

My setup for the Dillon dies - 9mm, 38SP, 40S&W and 45ACP. Posted many times here and on Enos.
Lyman decapping pins (7837786) - smaller that the dillon pins.

Take a broken Dillon pin, file the broken piece down and square, then reassemble the decapping rod as shown in the picture.
