Problems with my M14 today

Took my M14 out to the range in Stittsville today. went through 20 rounds of PMC with no problems but 3/4's of the way through my box of Winchester 150 gr.'s the op rod popped off the bolt lug and it wouldn't take the round out of the chamber. I cleared everything and took a look. Somehow the rod came out of it's small guide. Right beneath where you pull the op rod handle it rides in a little groove and it jumped out of that somehow. It went back in but I don't want this to happen every time.
Thank god nothing exploded like what happened to dogleg. Anybody ever had this problem?
 
Happened to me twice but only while I cycled by hand. My M305 only has 60 flawlessly fed and fired rounds. Seems solid all regards.
 
I was watching while my buddy was taking some shots and the brass wasn't flying as far as it normally does. The brass was only dropping 1-2 feet to the right instead of the normal 5 feet to the right and a bit forward. I seen the same results when I shot a few 180 gr bullets through it last summer.
 
The op rod guide on the barrel may be crooked by a very common problem of an incorrectly indexed barrel.
Just a thought, but is your barrel on straight?
 
You know what, It doesn't exactly look straight. Me and a friend got ours at the same time, My friends was definitely indexed improperly but mine didn't look bad. After staring at it for some time it does look like the sight blade is leaning slightly to the right.
Might be time for a trip to gunco.
 
It would explain why the op rod is popping out.

I'm not an expert, we'll see what the others have to say.

seen this on a few norcs with out of index barrels..... mainly barrels that are a few degrees past index.
on some it has been an ill fitting compatability between oprod tab and receiver groove..... solution...... new oprod, fit to mate with receiver. email dlask..... the only source for NEW oprods or get a good condition usgi replacement.

a "slightly" loose oprod tab to receiver groove fit can be minimalized by match conditioning the oprod guide so it can not move on the barrel...... this requires some detailed explanation....... so.... if you have an m14 capable smith close to you... no doubt he can do this in quick order.

I agree, but have to add that the op-rod guide may be loose as well. The barrel may be properly indexed but if you have a loosey-goosey op-rod guide, that will cause problems. It has been known to happen on 'norcs.

If the op-rod guide is loose on the barrel, tighten her up.
 
My last one was VERY loose and freely rotated about 15º because there was only 1/2 a roll pin in the guide. I used Loctite 641 and Hungry's 1/8" X 3/4" spring pin with a 5/64" X 1/2" spring pin drifted in the center of the other one. Very tight fit, the guide will not move at all and the fix costs about 22 cents.
