S&W model 41
Had the same problem with mine. I did two things to help cure the problem. First, make certain the chamber is not fouled with wax and powder. Second, remove the extractor and with a diamond file, file a slightly positive hook angle on the claw, such that only the claw tip grips the case rim. Do not file to a sharp point, leave about a .015 to .030 flat.
If this doesn't work, but I think it will, there is a little additional work you can do, but I won't describe it unless you really need it. When I completed my 41, it went through 50 rounds without one FTF or FTE. I haven't fired it in about three weeks now, but I'm confident I've resolved the problem.
