scoring sheets for each stage

it seems easier to enter the results per stage for everyone on one sheet.
Does anyone have a file they can send me or direct me to;

ie stage # Paper __ Steel__ Penalty Rounds
Name A, C, D, M, P, Procedural, Time


I could try and hack one together, but if anyone has a link to a really simplified form that would be great.

I think with smaller stages it would save time and paperwork, but with large stages It could cause problems as it one can lose track of scored targets.

Or the link to the actual sheets would also be great.
