Sks won't feed third round

Load the attached steel lined mag with five rounds. Will feed the first two rounds but won't feed the third. Bolt will remain in the open position. Manually close bolt, will feed round and will then feed the remaining two in mag. Tried it three or four times and every time won't feed the third round. It's a Russian sks, any ideas what the problem might be.
 
Post photos of your magazine's pinning method... there are/were at least 5 different methods employed over the past 5 or 6 years (I have examples of all 5). The newest method, which amounts to a small piece of bar stock riveted to the follower pivot point has been reported as problematic (too tight) for an unacceptably high number of members here on CGN. Fixes vary, but basically you need to find a way to loosen up the follower.
 
I was going to post a guess of a 'dent' or 'small bur' etc at a certain point in the mag that's just enough friction on the follower to stop the spring from pushing up another round, but not enough friction that you notice when slamming rounds in off a stripper clip?

maybe try pushing the follower in the mag down with your finger and see if you can feel it catching at some point... sand/scrub/clean/polish and try again?
 
Are you loading using strippers or by hand? I find once in a while if I load by hand the odd round will not feed or will stove pipe, but have never had that problem when I use the clips to load. I think my issue is proper alignment of the rounds as I push them down manually, whereas the stripper lines them at the back of the mag properly.
