Saw that happen in Cadets, on the big range at Vernon, with a C1A1. Guy in front attached his mag, pulled back the charging handle and let her go.

Rifle came back right over his shoulder; last one was right over my head.
REALLY nice that we were only loading twelve rounds!
Talked to the Armourer a short while afterwards and he told me in no uncertain terms that the FN C1A1 is a semiautomatic rifle which is incapable of automatic fire and that NO FN C1A1 rifle had EVER gone automatic on that range.
He had the thing stripped down on the bench when he said this, mind you.
Firing pin broken and jammed, of course.
Thirteen would have been really unlucky (for me) but it wasn't there... thankfully.
There were a hundred other guys there at the time, but I think it made the biggest impression on me, for some strange reason!
