Some of the older Howa's had a problem with the shape of the bolt sleeve. The shape was such that if the shroud was rotated so the firing pin was in the fired position, you could slam the bolt ahead (breaking or bending the safety) on a live round with the firing pin sticking out and discharge the round. The bolt then became a projectile rearwards...
...several years ago a real dummy did this exact thing...with a reload to boot...this happened here in Victoria with a Weatherby Vanguard (Made by Howa) . Weatherby settled out of court...
