What is Wrong? (Grouping)

So, went to the range to try some loads with a new powder. I chalked up the first flyer to maybe being me. Then a flyer appeared on the next group and the next and the next. What could be causing this?

All groups were done from a warm barrel.
 
It was raining the whole time. No real change in conditions (10 deg C, raining, light wind). The dry targets were just up on the board protected by a 2x4.

Kimber 8400 Advanced Tactical .308 Win
100 Meters
168 gr. SMK HPBT
Lapua Brass
CCI-BR2 Primer
Varget Powder

1st Target - 44.8 gr
2nd Target - 45.2 gr
3rd Target - 45.6 gr
4th Target 46.0 gr

The flyer would always appear in the first 2 shots. None were shot from a cold bore.
 
46 grains of Varget is on the hot side for 168's and is a compressed load. 42 grains is the starting load so you should start testing from there. I also see you are jumping up .4 grains with each load, you might want to try .2 gr increments to find the sweet spot. Velocity does not always equal accuracy and each rifle/barrel is different.
You also do not mention what type of rest you are using during testing or if you were using a rear bag with it.
