Where can I get a mount similar to this one?

American Defense MFG, some of the sponsors up top carry them.
One Shot Tactical and The Shooting Edge carry their stuff, not sure if they have the exact one you need, but ADM does make that size of mount:
And they are also a QR mount which is repeatable. ADM will ship their stuff directly to Canada, so if none of the site sponsors have them in-stock (or can order them for you) ADM can ship em strait up.

I have 2 of them on my Aimpoint Micro's and one on my Vortex Viper PST 4-16, and they're great mounts!
