I did a Google image search, and it turned out it's a Crossfire MK-1 12 gauge / .223 combo rifle, made by Crossfire LLC of LaGrange GA. It looks like the company went bankrupt a while back.

That crossfire gun had a serious safety recall IIRC. That's why they went out of business.
