Winchester Factory Ammo Malfunction (pic heavy)

Out hunting last night for geese with a couple of buddies and all of a sudden let off 3 rounds no issues. Reload first one goes off no problem all of a sudden hit the trigger again poof there goes the lead. Damn this isn't good quickly act to look see that the action is still open hold the gun away then off it goes flames out of the action. Brand new gun and ammo also. Here's the results. Any ideas what the problem is?

Double load? Not sure if that's even possible in 12ga.

Definately NOT overloaded... my guess is way underloaded, ... powder not compressed enough to BANG and just burned (explained by the flames and melted shell). THe fact the wad didn't make it out the barrel means only the primer went off at first, and the powder just burned.

Any chance the powder got wet? Was it right out of the box?

Either way... not a huge deal...

Ive never seen something like that before. Looks freaky but harmless, I guess.

Last year while hunting rabbits I had one 12 ga load of # 5, fart out the barrel, no bang or flop.

The bunny got away... I just reloaded.
