I was looking at a XCR-M today at the store and noticed right away that the handguard and barrel didn't line up. The barrel very obviously canted off to the side, which would make sighting in the rifle an interesting experience. I know that I've read of at least one such case, but how common is this problem? Is it a factory defect or a simple brain fart that was done during assembly?

I'd be questioning the installation of the barrel and tightness (or presence??!?) of the retention bolt.
FWIW, issues like these have affected SCARs too. Search google for "SCAR canted barrel", for example. I think if it were a big issue, it would be widely known (such as in the SCAR case)

There was a problem with slanted XCR barrels a few months ago. I believe there was a way to fix it yourself or Wolverine Supplies will fix that for you.
IIRC, it was just a canted gas block pressing against the inside of the fore-end.
