xcr-m handguard/barrel not in line

I was looking at a XCR-M today at the store and noticed right away that the handguard and barrel didn't line up. The barrel very obviously canted off to the side, which would make sighting in the rifle an interesting experience. I know that I've read of at least one such case, but how common is this problem? Is it a factory defect or a simple brain fart that was done during assembly?
 
This is the first I've heard of it on an xcr. I have seen it on a few acr's though, once where it wouldn't sight in, and once where it shot fine, barrel was just a bit off centre in the hanguard as opposed to not being parallel with it.
 
I'd be questioning the installation of the barrel and tightness (or presence??!?) of the retention bolt.

FWIW, issues like these have affected SCARs too. Search google for "SCAR canted barrel", for example. I think if it were a big issue, it would be widely known (such as in the SCAR case)
