AR15 feeding/bolt lock issue, need some help. :P

Hi, guys ,I am having some issues with my Dominion Arms AR15.
Mine has Magpul BAD level installed, it has a CQ-A upper reciever with 13.5 YHM rails and low profile gas block. All lower receiver internal parts are originally parts.

Issue 1:
Bolt won't lock open after last round. This happens a lot especially with PMAGS, and it also happens on LAR15 or standard mags but less. I can engage the bolt catch into lock position by hand with any mags, and seems nothing is blocking the bolt catch. I will do some test next time with MAGPUL BAD level removed to see if it makes a difference.

Issue 2:
When the bolt does locked back(most of time by hand), release the bolt always result to some kind of jam, either the bolt is stopped half way by the first round sitting in a 45 degree angle, or the bolt just went over the first round and close the chamber with nothing inside and leaves a scratch/dent on the first round. I have to use charging handle every time after each reload. However as long as the first round is chambered, I haven't experience any Jam during weapon cycling.

Issue 3:
The brass extraction direction is very inconsistent, first round is always tossed at 4 o'clock and then last round is always tossed at 2 o'clock, is this normal?

What direction should I look into to solve the above issues? Thank you guys.

Issue 2: Ditch the BAD lever. does it happen with all mags? pmags, lar, regular? Anti-tilt followers maybe?
Issue 3: Who cares where the brass goes. If you reload and want to save the brass, get a brass catcher.

Edit: Do you have the same problems with out the BAD lever and the original upper installed?

Thanks for the reply, I will test the gun with original upper this weekend just to make sure :p I guess after all BAD LEVEL does cause issues on some rifles.
 
It's not just the DA's - other rifles have he same issue with the BAD lever, you might say it's a 'bad' design. It's also mostly a gimmick, it won't make you into Travis or Chris.

Good point, I bought it because I thought BAD lever can be useful to lock the bolt back during malfunction, but if it is the source of the malfunction in the first place I better remove it.
 
Who installed the new quad rail on the cqa upper? The gas block might be misaligned.
I bought the upper from EE here, I will contact the seller for more information. I will test out the original upper this weekend the see if they make a difference or not.
 
Try removing the BCG and charging handle. Then slide the BCG back in alone, pay close attention to when the gas key on top of the BCG engages the gas tube. There should be absolutely no resistance. Are you able to post pics? I'd like to see the gas block without the hand guard installed as well as inside your upper receiver where the gas tube comes in.
