Keep in mind: disassembly with a dust cover mount is more work. Whereas these low profile mounts have a QD lever.

We sell the low profile alone for $75 USD, but if you need your side rail too they come together for $125

Yes, the side rail includes the screws to thread it into a CSA Vz58.

Can't speak for certain on the Cz858, FSN's, Czh2003 or other wild-cards that might have different receivers, but generally everyone is building the scope mounts the same.
 
I have ordered a ton of stuff from Zahal and found that they are EXCELLENT to deal with and often their orders have come faster than when I have ordered from Brownells in the USA.
 
Zahal just ordered mine, but after the (FRAUD ALERT) transaction was approved I was sent back to your site but it never progressed past the payment page to complete.
