Wtf? Has anyone seen this happen before? Is it a poorly cut port that I havent noticed before? Rifle has about 1200 rounds through it and has been cleaned and oiled thoroughly every time it has been used

It isn't damage. It didn't develop or form with use. It is simply a hole created when it was machined when produced. I have seen many like that right out of the cosmoline. It is normal. Remember it is dead air behind the piston face. The vent holes prevent pressure lock allowing the piston to cycle freely without having to compress the air in sudden bursts behind the piston head allowing the piston to cycle. Having another hole behind the area of piston face travel does nothing except allow faster displacement of air when piston cycles. The opening is created by manufacture, not failure.
 
Mine has the same 'feature'. Machined by the same guy? After a night of too much Vodka?

Either way it works. It for sure effects the gun though but Imo in a good way. My friend I went out with last weekend has a sks without this hole, his gun send the brass flying 20-30 feet, we were firing from 10 feet behind my 4runner and his cases were landing on my hood half the time lol. My sks just chucks them 10 feet or so.
 
Several people have posted about this mysterious 3rd hole before. I checked mine but no 3rd hole, maybe I just need a bigger flashlight and more lube. Lol
