1911 reliability problem

It's the first time i have this problem in my 1911
It's a double stack 40 sight tracker, but this doesn't make much difference

1 round out of 100 (literally), the extractor claw does not catch the rim.
The bullet is unfired.
The slide won't close completely as the extractor claw is stopped on the rear of the cartridge.
The only way to clear the jam at this point, remove the mag, point the muzzle 45 degrees up, and let gravity do the job.

I tuned the extractor to the max, i can't go further as the extractor has a lump a few thou behind the claw, that touches the extractor tunnel preventing it from being over sprung.

Should i be looking at my OAL (1.190" with 180gr JHP, which is common in double stack 40s)
Extractor?
Or mags?

I am usually handy with 1911s, but this issue been lasting weeks and i seem to always tune everything i just said, i am at a loss of ideas here.

I got provincials with this gun long sept. weekend, losing it at a smith right now is unthinkable, and shooting a big for me match with this will have me doubting all day.
 
When this occurs, can you seat the slide by giving the heal of the slide a smack of the hand forward (return spring weight?)?
Internal/external extractor?
Does it happen with all magazines?
What make of magazine?
Have you tuned the ejector too much? Is it dragging on the slide?
Is the gun lubed?
 
Sounds as if its mag related,the round is releasing from the mag lips too early and not being controlled by the extractor,check for adequate mag spring tension,check for cracks at rear of magazine opening up the feed lips,check dimension of feed lips that the lips haven't opened up(should be about .395 for 40 mag).Also check for excess lubrication within the mag causing the unfired round to move forward in the mag during recoil.

Never tried pushing the slide forward, the round was obviously not caught by extractor going up before chamber, and do not wish to break extractor claw

I have not touched the ejector, being a custom built gun it's the 1911 i have tinkered with the less as i beleive it should be already set optimal

Yep, propper 1911 lubrification is not an issue here. This is my 4th one, and have over 35K rounds in all 4 combined in the last 2.5 years

Yes you are on to something, rounds go flying out of mag when i drop partially loaded mag on cement floor.
I have went through all 6, one is indeed cracked at the base, not the others and have had this fault with all 6 mags.
Mag is dry as can be, as always.

Is .395 too big?
I am used to running single stack and double stack is new to me, in single stack i was always tuning to .350'', around .375''+ rounds would start feeding out of their own.
