I think I shoot the best with the medium one, but I prefer the feel of the largest size on my M&P 2.0

They definitely do affect your purchase, and therefor accuracy.

Shoot with whatever feels best!
 
Do you think the different sizes of back strap make much of a difference, or is it kind of a gimmick? What size do you prefer if you have one?

Id say it depends on how well the pistol grip fits your hand, and if you even require a different backstrap

Glocks come with back straps also and Ive tired them all but the standard grip with no added backstrap works the best for me

Nothing bad about having adjustability though even if I dont presonally use it I appreciate the options

As mentioned already, they absolutely make a difference in feel, it changes the grip size dramatically.

Correspondingly, it changes your grip, contact points, and trigger finger position. It's all subtle, but this can affect your base level accuracy and your shot to shot recovery. It also affects your ability to reach the mag release and slide release.

It's not obvious which one will be best for you: I shot IPSC with an M&P. When I first got the pistol, the largest backstrap "felt" best (I have large hands). However, after testing speed and accuracy with the different backstraps, I was surprised to see that I clearly shot better with the smallest.

After many thousands of rounds downrange, I eventually preferred the feel of the smallest grip. But if I were to get a different pistol with backstraps, I'd do those same tests again in a heartbeat. Maybe some people can "feel" which size results in the best biomechanics for them, but my feelings lied to me!
 
Adding onto alpining’s thoughts, once your grip technique is solid, you can be a better judge of grip sizes. I remember jumping on the bandwagon of flat aluminum grips for my Shadow and thinking they were great. Fast forward to when my grip improved, and I found the flat panels definitely aren’t for me. The VZ grips were much better because they filled my palms better.
