All is well

without handling your rifle, my best guess is the extractor has a burr or the extractor pivot pin is very tight. just a long shot.

Is the action smooth when running it without any ammo? Check the guide rods too, that they are properly in place in their holes within the upper

Might be weak-ish recoil springs, but I'd have a look at the chamber. If you have a bore scope all the better.
Put a piece of brilo pad on a rod and chuck it in a cordless drill and run it up the chamber to smooth it out if it is rough.

Mine had similar issues two days ago, when shooting in zero-degree temperature, with a couple of Pmags Gen 2. The fired round woud eject, but a new round would not chamber. It became a bolt-action gun. From time to time the fire round would not eject, and a new round would try to climb up the feed ramp, then the gun and mag got stuck. This had not happened when it was warm.

However the problem went away after I borrowed a metal 10-round magazine and tried again. All rounds shot, ejected and fed perfectly.

I will try again in a couple of days, and will bring different mags.

... I had failures to feed every 2-3 rounds and subsequently only about 1 per mag.

Gun troubleshooting 101 is that feed failures are usually a magazine problem. Tell us which ones you are using, and how many you have tried.

It's also possible that the magwell is improperly machined so that the mag is okay, but is in the wrong position relative to the barrel.

Don't see how

The bolt could be getting hung up on the mag lips. I had a 50/50 success rate with mags on hand, found that almost all issues I had were mag related. Some seat, but sit too deep and interfere with bolt movement. Or the rear of the mag forces the piece just below the bolt to sit weirdly..

I'd +1 for try other mags. metal Elander types worked best for me.

Interesting point. I have a couple more mag options to try.
