If this is not the case, check the bore with a good light.

It may look decent, but check the bore just inside the muzzle - the last 1/8th of an inch is the worst for getting rust, then copper, then lead buildup. If this is the case, chopping just a little and recrowning will make it shoot like new.
 
I've had keyholes from very nice bores under two conditions (note I use cast bullets):
1. The bullet is undersize for the diameter (in my case, the bullet was about .004" undersize for my .461" diameter 45-70 trapdoor).
2. The bullet was too much oversize. I found that bullets that were .003 oversize in a .45-90 I once had would tumble, but bullets that were sized at groove diameter or .001 over shot very accurately.

Note: these were individual cases that may not occur on your guns due to a variety of factors such as bullet weight, diameter-to-length ratio, jacketed vs. cast, etc.
