Colt Problem

Hey CGN's,
Today I borrowed a buddies Colt Ar-15 for a range date. Its an older model, (no forward assist) 10.5" barrel. I was shooting 55gr PMC and had nothing but problems, it wouldnt properly eject the casings 90% of the time, mashing them into the top of the receiver or double feeding the spent casing into the chamber with the fresh round. What causes this?
 
Maybe afew things "i cant say forsure with out looking at it, but here are afew causes

-Do you have a HH buffer?
-bad mag
-ejector is seized or the spring is weak
-worn or broken gas rings
-lack of lube or dirty
-loose gas key
-Also 10.5" barrels have had issue, thats why most people use 11.5"
 
Sounds like it's short stroking. What does your buddy shoot (ammo wise) out of it...?

He hadnt fired it in quite some time but he was feeding it from a stash of IVI 5.56, and Remington UMC .223. This was the first time it had seen the range in close to a year, prior to which I had cleaned/lubed it. It could be any number of things, I will try and get it back to the range this week after some TLC and with a different magazine. Thanks for all the suggestions so far.
