I have a Remington 700 VTR in .243. I get one inch 5 shot groups at 100 yds with factory ammo. Four of the holes measure maybe 1/4 or 3/8 of an inch group but the first round is always a flyer, maybe 3/4 of an inch or 1 inch away from from my nice little group. How can I eliminate that flyer?

If it is consistent, aim lower to compensate for the 'out'.
Pretty common with factory barrels which is why we use match barrels for competition.
Properly stressed relieved barrels usually put the first rd into the same group as the rest of the bullets.

The problem with the vtr is a very soft stock, bedding and posibly floating the barrel will help .

The steel is run through a heat cycle a few times which helps the grain structure in the barrel to align in a positive manner.
Same concept as applied in other industrial applications.
Some companies have offered cryogenic stress relieving with mixed results. Don't think there are any left doing the service.
I assume you have done this test with a cold but fouled barrel?
If the first shot is always out from a cold bore regardless of the condition of the bore, then just adjust for it when you aim.
Or do a 'fouler' then shoot for your group.

Shoot a 10 round group and see what happens. 5 shots doesn't really constitute enough of a group to be truly diagnostic. Then... shoot another and see what it looks like.
If you have but one errant shot in a 10-shot group, and the second group is tight, you can chalk it up to a cold bore, and all you can do is compensate. I suspect any bore that moves that much with one shot will string all over the place.

I took the gun to the range this morning. First shot from a cold fouled bore was one inch low, the next 3 shots were were an inch above the first shot, all 3 were touching.

Would you recommend I try free floating the barrel? Remington left a pressure point right at the tip of the fore end.

Free floating has never hurt any rifle I have done.
proper bedding is also critical to consistency.
Factory barrels are a mixed breed. maybe you can resolve it. Maybe that is something you have to live with.

the best advice here was obtunded suggestion to shoot a ten shot group. That will tell you where you are at. If you shoot 10 rounds and you have 1 flyer then you can compensate for it. If you have 2 then you can't.
