This is the Sheet that I use - gives me allot of room to make notes. I can then tape Chrono information and targets to the page and keeping the sheet in a three ring binder. This way one sees the history of your loads. What worked what didn't.

Yes I have a reload data sheet. It is printable and designed for data entry and storage on a computer. An Excel sheet that totals rounds fired. If interested pm me with your email address.

oh, now i see what you mean, don;t have any
I usually jot down a quick cheat sheet inside the ammo cases - nothing printable, though, just some quick shorthand. Then at the range, I'll make some notes on the targets after I shoot the groups, and when I get home I'll cut each target apart, and include a detailled write up on each group, noting the date, conditions, powder, charge, primer, OAL, bullet, and rifle used.
Eventually, I plan on scanning in each target (and I have hundreds) to my computer for a digital copy. Then I could come up with a clever spreadsheet mapping trends - how group sizes are correlated to each of the variables. Digital is a much better medium for that sort of analysis than paper. And most of my job requires lots of excel slicing and dicing of various data sets, so it's a medium that I'm much more comfortable with. Give me a nice coloured graph that I can add some data points to in real time, as I shoot more groups, yeah baby, I'm such a nerd
