So the cleaning and ammo varieties didn't work. I'm thinking that maybe the extractor hook might be over worn. Basically the empty casing ends up jamming sideways in the bolt right at the front of the discharge port. Is it worth changing that hook?

I eyeball it...
When the barrel is in the receiver, there are corresponding cuts. This is where the retaining clip fits. The cut in the receiver is bigger than the cut in the barrel. I eyeball the amount of barrel seen through the receiver's cut and make sure it's equal on both sides. It doesn't take much to make it finicky.
Did you ejector look ok? The part that touches the rim nice n' square?
I think I get what you're saying. Basically center the retaining clip on the groove and torque it down. There is a fair amount of play in there for sure.
