S&W 686 SS/.357mag locked up

Have a newly aquired 686 .357mag, cleaned it and took it to the range the pistol locked up on me twice, couldn't open the cylinder couldn't pull the hammer back, had to hold the cylinder release and gently tap it with a hammer to get it to open. Any ideas what could cause this, gun seems to be in really good shape could it just be ammo related.(this is my first revolver)
 
i can't move the extractor rod with my fingers so i assume it is tight(i don't know),looked for crap behind the extractor star all that was there is a little bit of oil from when i took the cylinder out and cleaned it no build up around the edges and the star sits flush.Cylinder release is on the side which moves what i guess is the internal lock that is in the center not on the side, if i move the cylinder release 1/16th" the hammer will not #### but when it was locked up i had the release all the way pushed forward and couldn't open it up without tapping on it.Extractor rod spins straight so i don't think it's bent.Whats the best way to check if it is loose
