You have the barrel off now ?

There is a Pin on the Extension, and a Notch on the Receiver, which part are you referring to by Tab ?
 
Please let us know when you get this fixed.
I'm in the 'waiting for stuff to get here' stage, myself. Sort of hoping that I don't run into problems like this that i will have no idea how to fix.


A buddy of mine recommended a NEA barrel, but I avoided their website like the plague.
Heard 1 too many stories about their junk to ever trust it.
The Upper could be the problem, but if I were a gambling man, I'd bet that as you said, the NEA barrel extension is the problem.


I hope you get this sorted out.
I would personally send that barrel back, and wait for something better to pop up. I recently found a 10.5 on the EE.
It was not cheap, but I'm confident that PSA milspec actually means milspec.
 
Make sure that the NEA Hybrid brake is on good. A buddy of mine put one on his AR good and tight, or so he thought. As he was doing his rapids, you could actually see the brake unscrewing itself. A new crush washer and some loc-tite solved that problem.

How much torque did you use tightening it down?

Seems like your rail is misaligned with the upper receiver as well
 
I already got a reply from NEA, they gave me prepaid postage to return the barrel. Customer service agreed it was a misaligned barrel extension due to 2 new employees.

I followed all instructions for torquing. Start at 35lb-ft then tighten to align the holes for gas tube, which took me to 45ish lb-ft. Gas block and gas tube were nice and straight at the 12 o'clock position. I should of added other photos on here to show that.

Thanks for the advice on the muzzle brake. Other than the loosening issue, did your friend like the brake?

I'll update when I get a new barrel.

I was thinking of getting one, but not sure about it. several 1 star reviews based on crappy barrel nut and warping threads or the rail being misaligned when tightened

I've had no problems with mine. The barrel nut is nice and solid and the top rail lines up nice with the flat top upper. I'll give more feedback when I actually get the rifle up and running. I don't see how the could be much difference from another that costs twice as much
 
Finally took it out to test, and it functioned flawlessly. No fail to feed or fire. A point was made earlier of the NEA Hybrid brake spinning off and it did happen to me.
 
2013-07-22 UPDATE

With the advice from an instructor, I replaced my NEA Hybrid Brake with a AGW Linear Comp and changed the standard buffer to a H2. All I can say is WOW!!! The recoil is like shooting a 10/22, just louder. I was tempted to sell it because I have other ammo hungry guns, but it is too fun to sell now.
