Wholly crap... that is, crap on my extractor and extractor tunnel!
I had some recent FTE problems with my Springfield 1911 so I decided to have a closer look at the extractor. After figuring out how to remove it and the firing pin I discovered that it was plugged with a solid black gunk! That is about 3000 rounds worth (since new).
Lesson learned. Include detailed periodic slide disassembly/cleaning as part of regular maintenance. Funny nobody else has posted a similar situation, I'll bet there are alot of dirty 1911's out there.
